- United Healthcare/Optum
If I am not an in-network provider for your insurance plan, my standard hourly rates will apply. Many health insurance plans will reimburse for “out of network” providers. I can help you to obtain reimbursement. Check your individual insurance plan for details.
*Please note that in order to bill insurance, I am required to provide them with clinical diagnoses, which will become part of your permanent medical record and subsequently classified as ‘pre-existing conditions” for insurance purposes. If you opt not to use your insurance benefits to pay for services, the standard hourly rates will apply.
